Homeless to Home Healthcare
Homeless to Home Healthcare Brisbane provides a variety of services to enhance the health of people who are homeless in Brisbane, and facilitate their transition to access housing and create a home including:
- The provision of primary health care and clinical services by a Registered Nurse
- Enhancing access to mainstream health care
- Linking and integrating access to housing, healthcare and support
- Hosting an accessible GP clinic
- Health promotion on critical health conditions
- Collaborative Support Plans with collocated agencies
- Collaboration with Queensland Health Homeless Health Outreach Team and Spiritus Homeless Program
- Access to Homeopathy treatments weekly
- Access and Referral to Counsellor.
Accessing health care services
Individuals and families can access Homeless Healthcare Brisbane through the following channels:
- Walk in to the BHSC
- Joint outreach visits with support and advocacy workers to rough sleepers and within homes
- Referral
- Phone appointment.
