Homeless to Home Healthcare

Homeless to Home Healthcare Brisbane provides a variety of services to enhance the health of people who are homeless in Brisbane, and facilitate their transition to access housing and create a home including:

  • The provision of primary health care and clinical services by a Registered Nurse
  • Enhancing access to mainstream health care
  • Linking and integrating access to housing, healthcare and support
  • Hosting an accessible GP clinic
  • Health promotion on critical health conditions
  • Collaborative Support Plans with collocated agencies
  • Collaboration with Queensland Health Homeless Health Outreach Team and Spiritus Homeless Program
  • Access to Homeopathy treatments weekly
  • Access and Referral to Counsellor.


Accessing health care services

Individuals and families can access Homeless Healthcare Brisbane through the following channels:

  • Walk in to the BHSC
  • Joint outreach visits with support and advocacy workers to rough sleepers and within homes
  • Referral
  • Phone appointment.
